1. I understand and accept that adopting an animal is a commitment for the lifetime    of the animal, perhaps as long as 20 years. If, upon further reflection, I cannot agree to such a commitment, I shall return this animal to All Creatures without further obligation.

2. I will take responsible, humane care of the animal, providing proper shelter,
nutrition, fresh water and gentle handling.

3. In case of illness, I will take the animal to a qualified veterinarian for diagnosis
and treatment. I will also provide him/her with necessary booster vaccinations.4. I agree to comply with all state & local laws and ordinances relating to the
keeping of this animal, including license and leash laws.

5a. CATS ONLY: I agree to keep this cat indoors. I agree to allow cat(s) to be
outdoors ONLY in the event the cat is on a harness, or in
a completely safe, cat-fenced patio or yard area which keeps other animals out
and this cat within the confined areas of the fencing. I recognize this condition as
being for the good health and safety of the cat.Under no circumstances will I have this cat declawed or mutilated in any other way.

 

5b. DOGS ONLY: If the need exists, I agree to take the dog to obedience training.

6. I understand and agree that the first seven (7) days are PROBATIONARY and the animal may be returned to All Creatures for ANY REASON, or reclaimed by
All Creatures for ANY REASON during the probationary period. The adoption
fee is refundable during this period. If he/she needs veterinary care during

this period, adopter agrees to take animal to All Creatures' designated veterinary hospital, or assume expenses if taken to any other veterinary hospital.

7. I agree to permit a representative of All Creatures to view and investigate
the premises where the animal is kept In the event that I fail or refuse to return
the animal upon demand, because any of the terms of this agreement have not been
met I agree to pay reasonable attorney's fees if an attorney is consulted, or a suit
is filed for its return.

8. I agree that if I am unable to keep this animal, I will notify All Creatures immediately.  I further agree to continue to care for the animal in my home until All Creatures can move the animal into a foster or permanent
home. I WILL, UNDER NO CIRCUMSTANCES, GIVE, PLACE OR TRANSFER
THIS ANIMAL MYSELF TO ANY PERSON(S) OR AGENCIES WITHOUT
FIRST OBTAINING WRITTEN PERMISSION FROM All Creatures.

 

9. I agree never to give or to sell this animal to another agency, or animal shelter,
or allow it to be used for experimentation, dog fighting, as a guard dog, or any
other commercial activitity. I am not an animal dealer, licensed or otherwise.
